Can I make appointments via email?

No, you must ring the clinic on 9382 9600 to make or change an appointment.

 

Can I contact my Practitioner via email?

Patient care will not be discussed via email. This is not only a legal requirement, but ensures patients receive the appropriate time and care from their practitioner.

 

Do I need a referral?

As at 1 November, 2010, a referral from a medical practitioner is recommended to see a Sports Physician at SportsMed Subiaco.  The reason a referral is recommended is due to recent changes to the Medicare system whereby a higher Medicare rebate applys to consultations where a patient has been referred by a medical practitioner.  You are still able to make an appointment directly or via a allied health practitioner (physiotherapist/podiatrist/pharmacist etc) however a lower Medicare rebate will apply for these consultations.  A new referral is required for each new injury/episode.

Please remember to bring any relevant information such as x-rays, scans or other medical investigations to your appointment.

Note:  A referral is not required to see a Physiotherapist.

 

Do I need to bring anything or wear anything in particular?

Please bring any x-rays, scans or other medical tests that are relevant to your injury with you to your consultation.  For ease of examination, please wear comforable clothing if possible. For orthotic assessment please bring your sports shoes and/or work shoes.  Ladies may benefit from bringing a small selection of your usual footware.

For Compartment Pressure Testing, you will need to bring clothes and footwear suitable for exercise.

 

Is parking available?

Yes, metered parking is avaliable on site (please see map under Contact Us).  Please bring change for parking machines and allow 20 minutes prior to your first appointment time to allow sufficient time to obtain parking and locate the clinic.

 

Do Sports Physicians specialise on particular parts of the body?

All Sports Physicians are trained to examine and diagnose all musculoskeletal and exercise-related conditions throughout the body.

 

Do I need to pay for the visit at the time?

Yes, payment is required on the same day of service.  For your convenience we provide EFTPOS and HICAPS facilities.  We also accept Visa and Mastercard.

Consultations and procedures with a Sports Physician may attract a medicare rebate to be claimed back by the patient.  If your bank details are registered with Medicare we can electronically submit the claim on your behalf.

For Physiotherapy services, we provide Hicaps for automated private health insurance rebates at the time of billing.  Rebates may apply for Physiotherapy services including Hydrotherapy, Ante-natal and Post-natal classes, Osteogym classes and Clinical Pilates sessions.

*** Medicare rebates usually only provide a partial rebate of the fee charged.  Please bring your Medicare card/details with you to your appointment.

 

Do you treat Workcover and MVIT Patients?

Yes, but please be aware you will be required to bring your claim number, date of accident and employer's full details including name, address and insurance company. Please note that all accounts are the responsibility of the patient and that Workcover and MVIT may only partially cover the cost of services at this clinic.

 

Is there a cancellation policy?

Yes. Please phone the clinic 24 hours prior to your appointment if you wish to cancel or change your appointment. A cancellation fee applies after this period.

 

Can I obtain a refund for Babyswim, Post-natal, Ante-natal or Clinical Pilates classes booked in advance?

No. We do not offer a refund for any classes booked in advance or purchases made in a block.
